Community support/outreach Community support is when workers or volunteers visit people in their own homes or meet with them in a public space such as a library, café or park. The intention is to help people stay connected to their usual places rather then engender social exclusion by only spending time in places associated with physical or mental ill health such as hospitals, clinics, day centres, health centres etc.
